Preparing for Your Visit

Insurance

Check your insurance to see if a referral from your primary care physician is needed.

Be sure to bring your insurance card, appropriate ID, and co-payment information.

Paperwork

Bring a list of all the medications you are currently taking. Also bring with you a copy of any reports of tests, x-ray films, or CDs.

Appointment

Remember to bring your own means of transportation and someone to take you home, if necessary.

Show up 15 to 30 minutes prior to your scheduled time. If you can't make the appointment or need to change the appointment time, please call 24 to 48 hours prior to the actual appointment.

Failure to notify the office for cancellation or not showing up for the scheduled appointment may result in charges.
